[EVA] The shrinking of adam

I have been watching the series over and over during the last few weeks. 
/post college graduation blues/....

Regardless, I understand that the second impact, was actually an attempt 
to shrink adam down, but in the episode where Kaji brings Gendo Adam, 
Adam is still rather large (I would guess 10cm diameter). How does Adam 
get shrunk down (or better said, where if at all is it explained) to 
being small enough to fit inside of Gendo's hand?

Additionally, if the souls of the EVAs are the mother's of the pilots, 
then is the 4th childs mother the soul of unit 04? I actually suspect, 
from the way it is discussed, that it is his sister, and not his mother. 
After they discuss 4ths (can't remember his name) sisters transfer to 
the NERV medical facilities, they never discuss her again. Any thoughts 
on NERV using her as the soul of Unit 04?
