I think it showed pretty clearly that Asuka's mother did indeed kill
herself. It never really showed /where/ she killed herself though, weither
it was at the hospital or what, though it probably was since Asuka was
running there to tell her mother about Asuka becoming a pilot.

It says very clearly in the manga that Asuka was made from "expensive
sperm" and has probably never seen her biological father. Also in the
manga it said Kyoko's husband had left her to be with another woman
because of Kyoko's difficulty in bearing children, so Asuka probably never
had a father figure while her mother was alive. So when she says she
doesn't need her papa she's probably refering to the fact she has never
had one.
