Not yet but I have noticed missing subtitles in episode 5 (vol 1).

Playing Episode 5 in Japanese, I can get no subtitles for the
loudspeaker announcements between 5m:18s and 5m:46s, i.e the whole
scene. The scene follows Ritsuko & Maya's "unstable mind" discussion
of why (orange) 00 rejected Rei in her capsule, after trying to get at
Gendou through by smashing his window.

It starts with only the announcement. Shinji, Misato & Ritsuko's
speech is subtitled, but the announcement remains untranslated
throughout. I will check, but am sure the announcements are translated
on the ADV VHS & orig DVD releases.

I tried on a Panasonic & a Pioneer player. Only 1 English subtitle
track works.
