Presumably it was extracted in a manner similar to how Lilith's was, but 
there is no real information on that subject. Kaworu has Adam's soul, and 
Gendo possessed Adam's body once he recieved it from Kaji earlier in the 
series, so at some point Adam's soul must have been removed from his body. 
Probably during the process that forced him back into an embryonic state.

Presumably that was done to keep Adam from awakening, since we see what 
happens to Lilith once she got her soul back in EoE. There's a lot of 
presumptions here, but that's because we never got an episode focusing on 
how Nerv/Gehrin/Seele did the dirty work in precise detail. Mainly because 
while many of us would find it interesting, fandom as a whole wouldn't.
