I believe it was mentioned at some point before volume 5 that Sachiel was
second, but I can't remember where exactly.

Also recall that in the last Stage, during the discussion about Pandora's
Box and whatnot, SEELE mentioned the Black Moon, but did not specify Lilith
as the progeniter of man as they did in the anime.

So for all we know, Sadamoto could be taking a slightly different angle with
Lilith.  I mean, I also found it suscpicious that Kaworu waited so long to
even start using the word "Lilim".  We'll just have to wait and see what
kind of explanation he throws at us when the time comes before we draw any
conclusions. But right now, there's really no grounds to claim that "12th
Angel" was a typo.
