Eva-01 and Eva-02 received their souls as a result of Contact Experiments 
for which Yui and Kyoko respectively served as test subjects. They put on 
"dive suits" and interface headsets, got into entry plugs, and the rest was 
history.

They don't provide any real information on how subsequent Evas are handled, 
aside from their ability to "prepare" a core for Toji, and Ritsuko's comment 
on how the Evas' souls were "salvaged". "Salvaging" technology lets souls be 
manipulated -- one vessel to another (e.g., human to Eva) or from no vessel 
to a new one (e.g., disembodied Shinji to new body).
