[EVA] Final release of original Eva?

I hope this doesn't add to any of the confusion of what's being discussed,
but where do the Video versions* (as opposed to the Theatrical versions) of
25' and 26' (EoE) fit into the picture of releases?
*The differences between the two: The video versions have the Next Episode
Previews of Air (seen after the end of 24' on Platinum/Renewal) and My Pure
Heart for You (just some footage of Gainax's studio(??) while Misato babbles
on about "Ikari Shinji-kun's final decision", I think it's done nicely
though.), Asuka's "iya." (No) being written instead of spoken, the eyecatch
having Neon Genesis Evangelion instead of The End of Evangelion, 26'
actually having credits, supposedly the Orchestra version of Jesu, Joy of
Man's Desiring... Since I haven't seen the video versions, I don't know what
other changes there are. I'm just going by MDWig's site. :P)
The only thing I know about those is there were released along with EoTV 25
& 26 on an LD(??), think it was Genesis 13 or some such thing? Supposedly
the order on the release, according to LD scans I saw online somewhere was:
25, then 25', 26, then 26', IIRC. Guess that means Death and the theater
version of EoE was released on a separate....thing. Speaking of which, WAS
the original Death ever released on Video/LD/DVD, or was that only
Death(true) and or Death(true)^2??
