In the TV series (no OAV ), Rei was injured in a accident that happened
when the EVA Unit-00 was test-piloted. (Spoiler)... ....It went beserk. In
the manga, if you look closely, it was Rei in Unit-01 attacking the Angel,
not Unit-00. But she was injured in it. A note, the anime is Anno's
interpretation of EVA, while the manga offers the point of view of
Sadamoto.

Reasons I don't know of. You'll notice too that throughout the series the
Fly Me to the Moon song has many versions played at the end. On the
soundtracks, there are even more versions. A whole lot more...

It was planned that way, the redone 25 & 26 be released as OAVs. But Gainax
decided to release them as Movies instead. So NO OAVs were ever released.
The first movie "Death & Rebirth" recaps the whole series and previews 25.
The 2nd movie "End of Evangelion" shows 25 and 26.

The official translation of A.T is Absolute Terror. But we shouldn't be
nitpicking names. What's important is what A.T field is...its the
impenetratable force-field that Angels have. But actually...(Spoilers)...
..
Humans are the 18th Angel, and the A.T field exists around our heart,
shutting out people from getting close to us.
