I've always prescribed to the point of view that all of Eva was nothing
more than a figment of Shinji's imagination, utilizing people from his real
life in a fantasy reality that he has constructed to get away from the
humdrum of his ordinary life.

I've also thought about what may have lead to the creation of this
psychotic state.  Considering everything leads down to Kaworu's death,
perhaps it may have something to do with the Kaworu of the real world in
which Shinji exists.  Could Shinji have possibly killed (deliberately or
accidentally) this Kaworu, and escaped into madness because of it?  The
possible reasons why could make an interesting post tv episode 26 fanfic in
itself.
