   On a very reliable website I heard that Anno announced that "Kaji was 
shot by either someone from Nerv personnel or a Seele agent".

the quote I'm talking about is in the Episode 21 summary under 
"checkpoint".

## Actually in the film book (where "checkpoint" is found) Anno
## was actually trying to be ambiguous. The "checkpoint" is like
## "While a lot of fans think that it was done by Misato, Director
## Anno raised the possibility that it could have been done by
## Japan government or Seele agent". 

## My feeling is that Anno was a bit taken by surprise by 
## the imagination of the fans (Misato killing Kaji), so
## he gave out his original idea. That is, he tried to 
## make Kaji's death a mysterious case, but *not that* 
## mysterious as some fans had thought. But Anno, never want
## to make things clear in any way, does not rule out 
## Misato outright. But I took his words as very close to
## saying he meant Kaji to be killed by some agents (though
## we would never know whose agent is that).
