My impression is that she allegedly died in an accident while test
piloting.  But her demise and body were never shown and some people
think she was "absorbed" into Unit 01 similar to what happened to
Shinji.  Regardless, it IS evident that her soul "haunts" Unit 01
somehow.  

Don't think there is enough evidence to suggest Gendou "arranged" for
Yui's "accident" to happen, though that probably would not be beyond
him.

Going slightly OT, when I first saw the subject this thread, I thought
you guys were parodying the "Intel Inside" sticker.  Still, a "Yui
Inside" sticker might look cute on your PC.  Another gimmick for Gainax
to market.  :)
