That shot is of Lilith and Eva-01. Lilith and Eva-01 were
attached to each other. This is probably how they "made"
Eva-01 and why Lilith has no legs. They made Eva-01
from Lilith's legs.

Also the liner notes says that this picture could represent
what Eva-01 was like when Yui disappeared...

If that is what happened, then that means they were
attached when Yui had her "accident" and could be
still attached when Rei was created (which might
explain how Rei got Lilith's  soul).
