The same question was raised in japan.anime.evangelion and I don't think we 
came up with a good answer.  I can tell you, though, that the shoulder 
pieces do not cover existing biological parts, the shoulder pieces are added 
on to fit the Evas for combat.  If you'll notice, the orange Unit00 does not 
have them, however, the combat fitted version, Unit00', does have them.  The 
shoulder pieces hold the progressive knives.
