Misato & all those who had a Rei visitation at point of death or after
are saved IMO.
If the last 2 TV episodes are to be taken as part of the ending (not my
choice), then Kaji, Misato, Ritsuko & Yui are all there.
Yui did say (in EoE) that each individual could choose whether to return
to human form or not.
