I haven't watched my taped-off-Japanese-TV tape for a
long time either, but IIRC, D&R was never broadcast on
Japanese TV.  IIRC, what was broadcast on Japanese TV
was "Revival of Evangelion", ie Death and then EoE. 
If I'm correct, then the fact that you did find
differences between the tape and the DVD would in fact
suggest that the Manga Rebirth part is in fact
Rebirth, and not a part of Air.  Hope that makes
sense...
